Referring a patient to Advanced Dysphagia Diagnostics (ADD) is fast, secure, and fully online. Our referral form lets contracted partners send a Mobile FEES or swallowing evaluation referral in just a few minutes. As soon as we receive it, an ADD endoscopist will reach out directly to schedule your patient’s evaluation.
Please note: For the best experience, complete the referral form on a computer or laptop. The form is also usable on a phone—just rotate your screen to landscape mode.

How To Submit A Referral
The referral form is organized into three short pages. Follow the steps below to complete and send your referral.
Step 1 — Enter Patient Information
On the first page, enter your patient’s information.
Step 2 — Select Advanced Dysphagia Diagnostics
On the second page, choose ADD as your referral destination:
Type “ADD Partner” into the search bar.
Select “ADD Partner” from the Name/Organization list.
Select Missy Pore from the Name/Organization list.
Step 3 — Complete & Send Your Referral
On the third page, finish the details and submit:
Enter the referring provider’s information and any additional patient details.
Select a scheduling priority — urgent or non-urgent.
Check the box to confirm you’re not a robot.
Click Send Referral.
A pop-up will appear with options to download the referral and patient instructions. This step is optional—you do not need to download anything for ADD to receive your referral.
What Happens Next
Once your referral is received, an ADD endoscopist will reach out to schedule the evaluation—no further action is needed on your end. After the FEES is completed, you’ll receive a detailed diagnostic report with findings and recommendations, and ADD can provide follow-up swallowing or voice therapy if indicated.

Do I need to download the referral or patient instructions?
No. The download pop-up that appears after sending is optional. ADD receives your referral as soon as you click Send Referral.

An ADD endoscopist will contact you to schedule the patient’s evaluation. Once completed, you’ll receive a detailed diagnostic report and recommendations.
